HIPAA is a set of federal laws and
regulations governing, among other things, the privacy of health
related information. One of the requirements is that we notify you
of how we use and handle your health information in what is called a
Notice of Privacy Practices.
You also must sign a form stating
that you have received a copy of this notice (you can simply read a
copy online or each Chair Car or Ambulance will provide a copy for
you). You only need to do this once. You can read, download, and/or
print our Notice
Here on our website. You are welcome to download
the signature form on the last page and sign it prior to your
transport. Your welcome to give that signed form to the crew or mail
it to North Conway Ambulance, Valley Transfer.
